Dual Credit Request Forms

These forms are needed every semester to be able to register for classes. They have to be filled out with the student's information and fully signed by the student, parent, and HS representative/HS Counselor). These forms also serve as an agreement between the student, the college, the high school, and the State of New Mexico.

 

Change of Schedule Form

This form is used to make changes to the student's schedule. In order to make changes to the schedule, the student must complete this form with student's information and class information. This form must have the student's and HS Representative/HS Counselor's signature. Dual Credit & Early College students' schedules cannot be changed without this form.
